Start with the latest actual records

A shared baby tracker can help organize these details. It cannot tell you whether your baby is well or decide what care they need.

Show where the current instructions live

Keep the latest instructions from your baby’s care team easy to find. Explain where supplies are and how the caregiver can reach you or the care team. Discuss care instructions directly, including anything the caregiver does not understand.

Use your clinician’s instructions for feeding, sleep, medicines and other individual care. This checklist does not set a care plan.

Agree on how to stay in touch

For appointment preparation, keep those questions in your pediatrician appointment checklist.

Use a short handoff note

Copy these headings into your own note: “Last records: ___. Current instructions are kept here: ___. Contacts: ___. Things to clarify: ___. Next update: ___.” Keep the note current after a visit or a change of plans.

Only share family information with people you intend to have it. Household helpers who are bringing dinner or doing laundry do not need access to baby records.

Choose caregiver access thoughtfully

Next Up lets you invite trusted caregivers to your private family page. Invited caregivers can see and update family records, including the diary. Your records are separate from Community and are not automatically posted there.

You can read this checklist without an account. Open the first-year planner to preview your baby’s plan before choosing to save it.
