A pediatrician appointment checklist you can actually use.

When you are tired, it is easy to forget the question you meant to ask. This short planning checklist gives you somewhere to start.

Confirm the practical details

Save the confirmed appointment in your baby planner. A suggested reminder is not a booking.

Write your top three questions

Start with the thing you most want to discuss. Add when you noticed it and one example, so you do not have to reconstruct everything during the visit.

  1. What would we like help understanding today?
  2. Are there records you want us to keep, and how should we share them?
  3. What is the next step, and who should we contact with a question afterward?

Use the list to support a conversation with your clinician. If a concern needs attention now, contact the care team rather than waiting for an appointment.

Gather what the office requested

Keep the requested hospital paperwork, insurance details and relevant notes together. Ask the office what applies to this visit; this is an organization list, not a medical schedule.

Daily records only describe what you entered. Do not fill in gaps by guessing. If you track feeds, diapers or sleep in the baby tracker, check that the times and notes are accurate.

Keep the answers and follow-up together

Write down the instructions you receive, ask about anything unclear and confirm the next appointment or task. If another caregiver could not attend, agree what they need to know and where to find the current instructions.

Try this note: “Our questions: ___. Instructions from the office: ___. Next step and date: ___. Who will do it: ___.” Avoid recording other patients or their information.

Save your next step

You can use this checklist in a notebook without signing up. To keep your own appointments and visit questions together, open Next Up, preview your baby’s plan and create an account when you want to save it.

For help preparing the daily handoff, use the caregiver handoff checklist.
